Family/Group Survivor Scholarship Application

Group/Family Scholarship Application Requirements

Each group/family application must designate one Primary Applicant who is responsible for ensuring that every individual listed on the application confirms their scholarship by the required deadline.

Failure to confirm a scholarship by the deadline will result in forfeiture of that individual's scholarship.

All scholarship recipients are expected to respond to requests for information and meet any deadlines provided.

If you or anyone in your group or family is no longer able to attend, please notify us as soon as possible so we can make the necessary adjustments to hotel accommodations and conference arrangements.

Lodging (if applicable)

Because scholarship-funded hotel rooms are limited, we ask family/group members attending together to share a room whenever possible. Only one scholarship-funded hotel room will be provided for an approved family/group sharing a room. Additional rooms are not covered unless specifically approved in advance.

Scholarship Confirmation & Communication

Scholarship recipients must confirm their acceptance by the deadline provided in their scholarship notification. If confirmation is not received by the stated deadline, the scholarship will be forfeited, and the recipient will not be able to participate under the scholarship. This allows the scholarship to be offered to another survivor.
